Complete and Return Application Forms
Forms may be picked up at the school office between 8 AM and 3 PM on days when school is in session. Included in the forms is one which must be completed by your child's physician.
You may also download the application forms here. These files will be in PDF format, meaning you will need the widely-available Acrobat Reader installed on your computer in order to read them. If you don't have the free Adobe Acrobat Reader, you
can download it by clicking this icon.

Arrange for Entrance Testing
Readiness testing is required of all students entering kindergarten. It is also required of new students entering any of the upper grades. Testing is a tool used to evaluate a student's academic performance. Please coordinate testing time with the school office.

Complete EdChoice forms for elementary grades (if applicable)
The application period for EdChoice is February 1 through April 13, 2012. You will be notified in July if your child is accepted for this scholarship. See the Financial Aid section below if you want more information on this program. Please note that if you miss the April 13th deadline, you will have to wait until the 2013-2014 school year in order to get this financial aid option.

Acquire Uniforms
Uniforms are required for all students. Please contact School Closet (614-237-0088) for more information. Gym uniforms are
simply grey sweatshirt and grey sweat pants. These can be purchased wherever it is
convenient.
